What Makes Up a Physician Report

March 2nd, 2008 by admin

Physicians make use of two primary methods when collecting a patient's information for diagnosis- they being a history examination and physical examination. Both these examinations combined together are known as a physician report. The history examination comprises of the patient's personal experiences and observations in his own words, while the physical examination includes a diagnostic process whereby the physician evaluates and determines the causes and reasons of the illness, based on the prevailing symptoms and observations. A skillfully compiled history examination of the patient supplements the physical examination and makes for a very useful physician report.

November 15th, 2007 by admin

When selecting your top 529 plan, your eye must be on three important things. First, check out whether the plan account can be opened directly, or whether you will need a broker. If it is a broker plan, the costs will be higher. Second, check how much the total expenses to open the plan come out to. You will have to pay the enrollment fees, the program manager fees and then keep up the plan with an annual maintenance fees. If you are looking at making some investment with the plan, then you must check out the maximum contribution limit, which differs widely from plan to plan. Also see how many investment options your plan offers you.
